This readme file will explain the required fields and field types used
for importing data into your CobraFormsOnline.com manager.

Employees.csv

n_first = Employee first name (required)
n_middle = Employee middle initial
n_last = Employee last name (required)
gender = Employee gender (required)
dob_mo = Employee Date of Birth Month (2 Digits, required)
dob_day = Employee Date of Birth Day (2 Digits, required)
dob_year = Employee Date of Birth Year (4 Digits, required)
ssn = Employee SSN (9 Digits, required)
address = Employee Address (required)
city = Employee City (Required) 
state = Employee State (2 Characters, required) 
zipcode = Employee Zipcode (10 digits,required) 
phone_1 = Employee Phone (20 char)
hire_mo = Employee Hire Month (2 Digits, required) 
hire_day = Employee Hire Day (2 Digits, required)
hire_year = Employee Hire Year (4 Digits, required)
emp_plan_medical = Employee Medical Plan (Must be the exact name of the employees medical plan or blank)
emp_plan_dental = Employee Dental Plan (Must be the exact name of the employees dental plan or blank)
emp_plan_vision = Employee Vision Plan (Must be the exact name of the employees vision plan or blank)
emp_spouse = Does Employee Have a Spouse on their plan ("Yes" or "No", required)
dependents = Number of Dependents (children) on Plan (1 Digit, required) *set to 0 if none

emp_spouse_n_first = Spouse first name (required)
emp_spouse_n_middle = Spouse middle initial

emp_spouse_dob_mo = Spouse Date of Birth Month (2 Digits, required)
emp_spouse_dob_day = Spouse Date of Birth Day (2 Digits, required)

emp_spouse_plan_medical = Spouse Medical Plan (Must be the exact name of the spouses medical plan or blank)
emp_spouse_plan_dental = Spouse Dental Plan (Must be the exact name of the spouses dental plan or blank)
emp_spouse_plan_vision = Spouse Vision Plan (Must be the exact name of the spouses vision plan or blank)

dep_address = Dependent Address (if different than employees)
dep_city = Dependent City (if different than employees)
dep_state = Dependent State (2 Characters, if different than employees) 
dep_zipcode = Dependent Zipcode (10 digits, if different than employees) 
dep_plan_medical = Dependent Medical Plan (Must be the exact name of the dependents medical plan or blank)
dep_plan_dental = Dependent Dental Plan (Must be the exact name of the dependents dental plan or blank)
dep_plan_vision = Dependent Vision Plan (Must be the exact name of the dependents vision plan or blank)
dep_number = Number of this dependent. Number each employees dependents starting at 1.  Each employees dependent must have a unique dep number starting at 1.
